PLLC, does not currently accept insurance for psychological assessment. We are in the process of becoming paneled with multiple insurance companies so check back frequently for the most up to date information.***

Any fees for services provided not covered by the insurance company will be your responsibility. You are responsible for paying applicable co-pays, co-insurance amounts, or deductibles at the time of service.

The following listed fees are for individuals paying out of pocket (not using insurance) —

    • For individuals -

      • Initial/Intake - $150

      • Subsequent Sessions - $125

    • For couples/families -

      • Initial/Intake - $175

      • Subsequent Sessions - $150

  • At Lee Therapy and Assessment, PLLC, we realize that seeking comprehensive psychological services represents a significant investment of not only time, but also money. All costs will be explained upfront, allowing you to make informed decisions concerning your care. While our fees are outlined below, don’t hesitate to contact us to discuss options for covering the cost of services. Together we will come up with a solution that is best for you.

    Psychological assessment is unique when compared to the costs of psychotherapy. The cost for a psychological evaluation will vary depending on your needs. The cost of a full evaluation typically ranges from $900 to $1800, based on the fees below.

    • $225 for the initial consultation / intake appointment (1.5 hours).

    • $150 per hour of services. This includes face-to-face test administration, scoring and interpretation, report writing, and a feedback session. This does not include the initial consultation.

    Fees are due at the time of service.

    At the initial consultation, you will be provided with an estimate of hours needed for your specific type of evaluation. If the evaluation takes less time than estimated, you will pay less. If the evaluation takes longer than estimated, you will only pay what has been estimated.

    All fees are due at the time of service. We do not offer payment plans at this time. If you choose to discontinue testing for any reason, you are still required to pay the full fee for testing, based on the hours of service.

We offer a sliding scale fee on a limited basis depending on individual client needs. For questions or to see if you qualify for a sliding scale fee, please contact us.

Cancellation Policy

Out of courtesy to our clinicians and other clients who are waiting for an appointment, please call as soon as possible to cancel an appointment. At Lee Therapy and Assessment, PLLC, we understand the nature of emergencies and will work with you as long as things are communicated clearly and in a timely manner. However, appointments may book far in advance and, at times, are difficult to book with short notice.

For therapy — Appointments made after the initial intake session must be cancelled with 24 hours’ notice. If you are unable to reach the office, leaving voicemails, e-mails, or texts timestamped before the 24-hour time frame are not charged. Appointments that are not cancelled within 24 hours, or appointments that are not upheld (no shows) will be charged at our full rate of $125.

For psychological evaluations — Subsequent appointments made after the consultation visit must be cancelled with 48 hours’ notice. If you are unable to reach the office, leaving voicemails, e-mails, or texts timestamped before the 48-hour time frame are not charged. Due to the nature of psychological testing, 4-hour windows are generally set aside for the evaluation period. Due to the longer time frames set aside for these appointments, no-shows and late cancellations will incur the following fees:

  • Cancellation in less than 48 hours of appointment – $200 fee

  • No-show for appointment – $250 fee

 All fees listed above must be paid prior to any re-scheduling of the missed appointment. 

Good Faith Estimate

Beginning 01/01/2022 under sec 2799B-6 of the Public Health Service Act, behavioral healthcare providers and medical facilities are required to provide individuals whom aren’t enrolled in an insurance plan or a Federal healthcare program with a “Good Faith Estimate” (GFE) explaining how much your care will cost.

  • You have the right to receive a GFE for the total expected cost of any non-emergency items or services. This includes costs like medical tests, prescription drugs, equipment and hospital fees.

  • Make sure your provider gives you a GFE in writing at least 1 business day before your service. You can ask your provider, and any other provider you choose, for a GFE before you schedule a service.

  • If you receive a bill that is at least $400 more than your GFE, you can dispute the bill.
